Use this form to register or update your company as a partner of Finlandia Pharmacy and Natural Health Centre, and share the essentials we need to set you up — all in one place.

It begins by asking how your company works with Finlandia — as a vendor (brand owner / manufacturer), a supplier (distributor / wholesaler), or both — and then shows only the sections relevant to you. Brand owners can list one or several brands; distributors can list the vendors and brands they carry. You will not be asked for individual product details at this stage.

Submitting this form does not constitute product approval, supplier approval, or a commitment by Finlandia to purchase products or services. Our team will review the information and contact you about next steps.

Any information you provide is treated as confidential and is accessed only by authorized personnel involved in purchasing, accounting, compliance, and vendor management. Fields marked * are required.

Before you submit

• Your legal company name, company type, and tax / business registration numbers
• Your brand name(s) — or, if you distribute, the vendors/brands you carry
• Key contacts (sales, accounts receivable / remittance, owner)
• Ordering, freight, and returns / RMA terms (minimums, lead time, back-order, restocking)
• Compliance details (site licence, GMP, product-liability insurance) and payment / remittance preferences
• Any certificates / site licence / insurance / price list to upload (up to 10 files, 10 MB each)
